If you haven't got the discipline to stand by your goals then get someone who can support you. Someone who will remind you that you need to do it. Someone like your spouse, your children or a close friend. This will make them feel important too.
Reward yourself once you've achieved these goals or punish yourself if you do not achieve them. For example may be you have to treat your friends to a fancy dinner everytime you fail to achieve one goal. This way you'll be more motivated to take action.
